Welcome! You’ve made an incredible step to take control of your career and health. I’m extremely excited to begin working with you. Before we can begin, it’s important for me to learn, and for you to reflect on where you are now and what you’re hoping to achieve. Please complete the entire form. Once the form is complete, you’ll receive an email to set up your first coaching session!!! 🎉
Important: You must do this intake form in one sitting. There is no ability for you to save your work and come back to it. Please carve out 20-30 uninterrupted minutes, to complete the entirety of the form.
Definitions: When the form refers to health it means “physical, emotional and mental” health. For all other terms please use their common sense and reasonable meaning.
How best to be present while completing the intake form:
Schedule 30 minutes in your calendar;
Find a place where you will not be interrupted (make sure others know not to interrupt you during this time);
But anything away that could possible disrupt you, and place your phone on airplane mode;
Read through all the questions first before answering. Then take some time to think about them before starting the intake form;
When you’re ready to start typing, take two deep breaths to calm down and center; and
Begin! Let it all out. This is the time to be long-winded. Don’t forget to have fun!!
Privacy Notice: This Intake Form is private. No information will be shared outside of Citrine Consulting, LLC its employees and assigns, unless otherwise required by law. The intake form will be destroyed in its entirety no later than three years from the date of its completion.
